Oh gosh, I remember finding this when I was in high school. It actually bugged me that they were playing Hello differently to the recording. Lol.

I used to know Casey (the guys playing the piano) but we have since lost contact. His friend, Jake, has since sold the piano (on eBay) he bought because he and his wife were expecting and needed room.

Two interesting things that we learn from this video:
Amy is/was willing to play Hello live, since she says they haven't played that song live "yet" (1:30 - 1:40). It is the only song from Fallen to not have been played live at least once - not including the cello part of Hello that was often included in the Intro to the Fallen Tour spanning 2003.
Amy did not name the song Eternal (6:35 - 6:45). This is most likely because Eternal was made by Ben and David. Although since Amy eventually remembered the piano piece, and since she shares BMI credits with Ben and David, it is possible that she wrote the piano piece and Ben/David did the rest.

^The Middle part seems to be her style. The rest seems to be someone else's (either Ben or David) style. Though, this is all up in the air if/when/until someone confirms who wrote what.

From what I've gathered, Amy is scared to death of the low end of the piano, hehehe... Only going down there when she means serious business (TOS) or is seriously in love (Good Enough)... Or seriously depressed (TLSIWOY!) Hehehehe......... Anything high and twinkly is likely hers and god forbid the girl use a chord. Every once in a while she'll throw one in to remind everyone she has more than two fingers, but... There is definitely a very different style from hers throughout much of Eternal.

It's not. Amy just doesn't remember it. lol. Or maybe it was an untitled EP with just the band's name on it. I personally do not think the 1998 EP had a title. It was probably just referred to as the Evanescence EP and not titled that. The 1998 EP is indeed real. PM FaixaFive and ask him. He has an authentic 1998 EP where he talked to someone who is/was involved with the band at the time the EP was being worked on and sold.
